We are always available for after-hours emergencies. Dr. Hilton or Dr. Diminick can be paged by following the instructions given when the office telephone is called.

Completing the necessary forms in advance of your first appointment will expedite your visit. We have several forms that are used in our office. They are available here to view and print to bring with you. Note: You must have Acrobat Reader to open the forms.

Everyone must fill out the Privacy Acknowedgement Form.

If you are over the age of 18, you will fill out the
Adult Information Form.

If you are a parent bringing your child for treatment, you will need to complete the Child Information Form.
